Winner will be announced during the Classic Country with Eddie Bear program, Sunday, September 12, 2010.

You may only submit the survey once, multiple entries will be discarded and participant disqualified.

There will be no substitution or cash alternatives for prize.
Prize can be collected locally.  Failure to collect the prize constitutes forfeiture of the prize.
Winner is responsible for any postage fees incurred should they wish the prize to be mailed.

Winner will be contacted via the e-mail address supplied on the survey.  Eddie Bear Productions is not responsible for misdirection of mailing and accepts no responsibility for any situation that may or may not have been caused by the fact that you won a prize

Winner must contact Eddie Bear Productions within 14 days of becoming a winner to set up prize pickup or mailing instructions.
If Eddie Bear Productions is not contacted within this timeline, prize will be forfeit.
